There is no place like home and Home Care On-Call (HCOC) is the first free-standing, comprehensive electronic directory of communities served by visiting nurse association (VNA) home health care providers in the Northeast. Home health care services facilitate an individual's return home from other care settings and assist in regaining and maintaining health and independence. HCOC is easy to use and more current than paper directories. It also provides links to the web sites of many of the participating providers.

Remember! Home health care services cannot begin without a physician's order. Individuals being discharged from a health care facility should request a referral through the facility's social worker/case manager.

To expedite a telephone referral, please have the following information (click here) available before you call an agency. If you have insurance, be sure to determine what home health care benefits are offered through your health care policy. Call your health care administrator if you are unsure.
